Final Report Instructions

Due: upon completion of grant or by June 3, 2024

Final Reports are very important to the Foundation for two reasons.

  1. To measure and document the success of your grant
  2. To provide us with information and material to use in our newsletters, social media posts and other communications with current and prospective donors. The funding for your grant would not have been possible without the generous support of our community and we want to share your success stories to help secure continued support to fund future grants.

How can you help?

  • Provide thoughtful responses to the questions
  • Include meaningful quotes from students (if applicable)
  • Upload photos and videos of your grant in action*
  • Upload photos of the final product / work samples*

* Please ensure photos / videos uploaded only include students authorized to be photographed (edit/crop as needed)

Log into the Grant Application portal at If you forgot your password, click on “Forgot Password” and follow the prompts.

Once you are logged in you will see the below screen which shows the status of your grant application. Click “Submit Final Report” for the respective final report you wish to fill out.

You will land on the following page and fill out all sections of the final report (not all pictured below) and will also be prompted to upload photos. Photos are greatly appreciated.

Please be sure that all students are authorized to be photographed (crop/ edit as needed).

IMPORTANT - Save your work as you go. You can save and log back in if you need more time. Once completed, click submit final report.

You will receive an email with your final report and you will see the below notification to let you know it has been submitted.

Once your final report has been submitted, you can view the final report by clicking the button “View Application & Final Report”. To view your final report, scroll past your application to view the final report.
